Information for patients:

1. Where is the clinic? How do I get there?


Our closest major intersection is Leslie and Sheppard. The clinic is located on the east side of Provost Drive - just past the McDonalds and across from the backside of Canadian Tire. The clinic is on the ground floor of the Discovery Condo building (along the inner courtyard) and is attached to the Roya Boutique Pharmacy (visible from Provost Drive).


If you are planning on using public transportation - our closest subway stops are Leslie Station and Bessarion Station - on the Sheppard Line. The clinic is located between the two stations and is approximately a 10 minute walk.

2. Is there parking nearby?


Parking is available on the street (paid) or underground (complimentary). To access the condo underground parking (coming from Sheppard), turn left off of Provost Drive (just past the McDonalds) onto Singer Court. Look for the Discovery Condo Visitor Parking sign (on the right) - use the intercom and let security know that you are here for the medical clinic. Once the garage door opens, drive down the ramp and watch for the retail parking elevator entrance on the left - which will bring you up to street level just outside of the clinic.


***In order to use the underground condo parking, patients must park in the "c-spots" only. This is painted on the ground before you drive into the parking spot.

3. Are there any doctors accepting new patients?  (Please note that we do not keep a waitlist)

4. Do you see patients on a walk-in basis?

No we do not - appointments are booked and are offered to registered patients of the clinic only. Please note that these appointments are for urgent matters only. Follow up appointments, referrals, physicals and test results are appointments that should be booked with your Family Doctor in advance.

Saturday 9:00am - 3:00pm


** please be aware that due to high patient volume, appointment slots may fill up before the indicated times above.

5. Where is the closest imaging and lab facility?


There is a Lifelabs and York Radiology located across the street from the clinic at 1100 Sheppard.

6. Are animals allowed in the clinic?


Discovery Family Health welcomes service animals only.

7. Do I need OHIP coverage to be seen at the clinic?


In order to enrol with one of our family physicians, patients are required to be covered by OHIP. However, if a patient is either not covered by OHIP, covered by another provincial health plan or by third party insurance they can be seen but will be charged for the visit.


EXPIRED OHIP CARDS

Please ensure that your OHIP coverage is kept up-to-date at all times. If your OHIP is expired or due to expire soon, please visit your closest Service Ontario. The Ministry of Health will generally send you a reminder notice approximately two months before your OHIP expires.

8. Are there services not covered by OHIP?


Yes there are. Most basic services and appointments are covered, however certain forms, letters, and cosmetic procedures may result in a charge to the patient. Additional charges are set by the Ontario Medical Association (OMA). For more information, please speak with reception or your doctor.

9. What languages are spoken at the clinic?


English, French, Farsi, Hungarian, Cantonese and Mandarin


10. What payment options are available for non-OHIP covered services?


1) Credit card payment by phone - Visa or Mastercard (no information is written down, it is entered directly into the machine)


2) E-transfer to: payment@discoveryfamilyhealth.com
